Required Forms
ALL new elementary students are required to have the State-mandated green immunization form (available at physician's office and health department) on file in the office by the first day of school, along with copies of birth and Baptismal certificates.

All new pre-school students are required by state law to have an immunization form on file before the start of school.  We also require all pre-school students to have a copy of the Birth & Baptismal certificates on file.

Registration fees:
     1st Child:      $40
     2+ Children:   $50

Print out BOTH of the following forms, fill them out, and mail them to us with your registration fee:

• Registration Form -   Family Detail
  Fill out one per family.

• Registration Form - Student Detail
    Fill out one per student.
